OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to contact with anhydrous ammonia from potentially catastrophic releases from the refrigeration system of a warehouse used for the storage and distribution of seafood that is not effectively inspected, tested, and maintained to ensure its continued serviceability:   (a) Location - 1 Technology Drive, Peabody, MA:  On or about 08/18/2020, the facility's ammonia refrigeration system piping was not inspected at an interval sufficient to prevent excessive corrosion. Piping on the system's condenser was observed to be corroded, and subsequent non-destructive testing confirmed that it was in need of replacement.  (b) Location - 1 Technology Drive, Peabody, MA:  On or about 08/18/2020, the facility's ammonia refrigeration system piping has not been inspected at an interval sufficient to ensure its labelling is maintained. Refrigerant piping located within the warehouse was observed not to be labelled in accordance with the employer's chosen practice.  (c) Location - 1 Technology Drive, Peabody, MA:  On or about 08/18/2020, an inspection, testing, and maintenance schedule has not been established for the pressure vessels in the facility's ammonia refrigeration system.  (d) Location - 1 Technology Drive, Peabody, MA:  On or about 08/18/2020, observations of the operating condition of the facility's ammonia refrigeration system are not made and recorded frequently enough to detect and/or prevent departures from normal operating conditions in a timely fashion.
