29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) At the east end of Building 8, employees used a Snorkel TB60 telescopic boom lift, serial #OT06075RBLT, and were not attached to the basket of the lift, exposing employees to fall hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: a) At the east end of Building 8, employees worked on the roof at an approximate height of 20 feet unprotected, exposing employees to fall hazards.
