(a) Phase One Landscapes, LLC at 2316 Links Place in Erie, CO 80516: On and before August 14, 2020, the employer did not provide silica hazards awareness training to employees who were dry cutting stone blocks. This condition exposed employees to respiratory hazards related to silica dust. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.134(k)(1): The employer did not provide effective training to employees who were required to use respirators: (a) Phase One Landscapes, LLC at 2316 Links Place in Erie, CO 80516: On and before August 14, 2020, the employer did not provide effective respirator training to employees who were dry cutting stone blocks without wearing the disposable HEPA masks provided by the employer. This condition exposed employees to respiratory hazards related to silica dust. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
