29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a) Mountain States Packaging, LLC at 645 W 53rd Place in Denver, CO 80216:  On or about August 18, 2020,  the employer failed to provide machine guarding on the cardboard baler machine.   This condition exposed employees to a finger amputation hazard in that an employee had the first knuckle of his left index finger amputated when the machine activated.    Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
