Philadelphia, PA - 19146 - The employer did not ensure that a safe means of egress was provided within 25 feet of lateral travel for employee working in an excavation, approximately 57.3 ft. L x 15.3 ft. W x 6 ft. H, thus exposing employee to hazards associated with emergency escape.   Condition was observed on or about 19 August 2020.
