29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy had not been created and implemented:  a)    Kingsman Building S1 Room: On or about 07/09/2020, the energy control procedure did not specific how and what to use to secure the energy generated from the building air supply that caused the return fan #11 pulley to rotate, as a result an employee's right index finger was amputated.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date.
