29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  a) Construction site, Foxwood Ridge Units 178-185 - On or about August 20, 2020, Employees engaged in residential framing activities, were exposed to serious injury from debris that created trip hazards and lumber with protruding nails.

29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:  a) Construction site, Foxwood Ridge Units 178-185 - On or about August 20, 2020 Employees performing residential framing used a portable generator with a 5 gallon gas can for refueling, but did not have a fire extinguisher on site.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a) Construction site, Foxwood Dr. Units 181-183 - On or about August 20, 2020, Employees performing roof framing and sheeting at a height of approximately 20' above grade were not protected from falling by any means of conventional fall protection or alternative fall protection plan.   Lifetime Contractor Corporation  was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1472773, citation number 01, item number 002 and was affirmed as a final order on 7/2/2020, with respect to a workplace located at 5267 Foxcroft Drive Schnecksville, PA.  Lifetime Contractor Corporation  was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1470409, citation number 01, item number 003 and was affirmed as a final order on 6/17/2020, with respect to a workplace located at 10 Harrison Drive building 700 Northfield, NJ 08225.  Lifetime Contractor Corporation  was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1466396, citation number 01, item number 002 and was affirmed as a final order on 6/16/2020, with respect to a workplace located at 195 Shady Brook Dr. Langhorne, PA 19047.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a) Construction site, Foxwood Dr. Unit 179 - On or about August 20, 2020, Employees were utilizing an extension ladder with broken anti slip safety feet to access the elevated work area.  Lifetime Contractor Corporation  was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1470409, citation number 01, item number 007 and was affirmed as a final order on 6/17/2020, with respect to a workplace located at 10 Harrison Drive building 700 Northfield, NJ 08225.
