29 CFR 1904.7(b)(3):  When an injury or illness involves one or more days away from work, the employer did not record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away, and did not enter the number of calendar days away from work in the number of days column, or if the employee is out for an extended period of time, the employer did not enter an estimate of the days that the employee will be away, and update the day count when the actual number of days was known:  A.)  George Junior Republic dba George Junior Republic in Pennsylvania, 233 George Junior Road, Grove City, Pennsylvania  16127, on or about August 28, 2020 and times prior thereto:  The employer did not classify Case No. 2020-64 on the 2020 OSHA 300 log as "Days away from work" in column H for an incident that occurred on August 12, 2020.  B.) 29 CFR 1904.29(b)(1):  The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300:  A.)  George Junior Republic dba George Junior Republic in Pennsylvania, 233 George Junior Road, Grove City, Pennsylvania  16127, on or about August 28, 2020 and times prior thereto:  Column F for Case No. 2020-62 on the 2020 OSHA 300 log provides the object/substance that directly injured or made the person ill, but does not provide the injury or illness, or the part of the body affected.    B.)  George Junior Republic dba George Junior Republic in Pennsylvania, 233 George Junior Road, Grove City, Pennsylvania  16127, on or about August 28, 2020 and times prior thereto:  Column F for Case No. 2020-61 on the 2020 OSHA 300 log provides the part of the body affected, but does not provide the injury or illness, or the object/substance that directly injured or made the person ill.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR  1904.39(a)(2):  The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  A.)  George Junior Republic dba George Junior Republic in Pennsylvania, 233 George Junior Road, Grove City, Pennsylvania  16127:  An employee was hospitalized following a workplace injury on August 12, 2020.  The hospitalization was not reported to the OSHA Erie Area Office within 24-hours of the hospitalization.  The condition was noted on or about August 19, 2020.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
