At a jobsite located at 5432 North Waukomis, Kansas City, Missouri 64151, employees were exposed to serious eye or face injuries. Employees conducting framing work were exposed to flying particles without the use of adequate eye or face protection.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  At a jobsite located at 5432 North Waukomis, Kansas City, Missouri 64151, an employee was exposed to a serious fall hazard. An employee was observed operating a JLG 600s telescoping boom lift and was not wearing a harness or body belt with a lanyard attached to the boom or basket of the lift.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  At a jobsite located at 5432 North Waukomis, Kansas City, Missouri 64151, employees were exposed to serious fall hazards. Employees installing roof decking on a low slope roof were not protected from falling by use of adequate fall protection systems. Employees were exposed to fall hazard distances between 11 and 16 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a jobsite located at 5432 North Waukomis, Kansas City, Missouri 64151, employees were exposed to serious fall hazards. Employees conducting framing work were accessing an upper landing by utilizing a portable ladder. The portable ladder's side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. Employees were exposed to fall hazards between 9 feet 9 inches and 15 feet 11 inches to the ground below.
