Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. At a jobsite located at 5432 North Waukomis, Kansas City, Missouri 64151, employees were exposed to serious fall hazards. Employees installing roof decking on a low slope roof were not protected from falling by use of adequate fall protection systems. Employees were exposed to fall hazard distances between 11 and 16 feet to the ground below.

The portable ladder's side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. Employees were exposed to fall hazards between 9 feet 9 inches and 15 feet 11 inches to the ground below.
