29 CFR 1910.272(k): Receiving pit feed openings in the grain handling facility, such as truck or railcar receiving pits, were not covered by grates which had a maximum opening width of 2.5 inches: On or about August 26, 2020, the employer did not ensure that the receiving pit grate cross bars were constructed and maintained in a manner to ensure openings did not exceed 2.5 inches.
