29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. On or about August 25, 2020 and prior, employees performing maintenance on the electric forklifts were exposed to corrosive liquid when adding water without an eye wash station in close proximity.

29 CFR 1910.243(c)(3): Vertical portable grinders. Safety guards used on machines known as right angle head or vertical portable grinders shall have a maximum exposure angle of 180 deg., and the guard shall be so located so as to be between the operator and the wheel during use. Adjustment of guard shall be such that pieces of an accidentally broken wheel will be deflected away from the operator. (See Figure P-4.) (For Figure P-4, http://www.osha.gov/OshStd_gif/10PFP_4.gif) (a) In Building 4/Paint Shop, DeWalt 4.5 inch portable angle grinder used to cut product did not have a guard exposing employees to struck by hazards. (b) In Building 4/Paint Shop, Milwaukee 4.5 inch portable angle grinder used to smooth product did not have a guard exposing employees to struck by hazards.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. The electrical sit down forklifts were not inspected on a daily basis to ensure no conditions adversely affect the safety of the vehicle.
