29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) 1754 Gibson Rd. Salem Manor Bensalem, PA - On or about August 21, 2020, employees were performing roofing work on a steeply pitched roof, 30 feet above grade, without utilizing an appropriate means of fall protection. McCann's Contracting, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard , which was contained in OSHA inspection number 1425615, citation number 1, item number 1 and was affirmed as a final order on October 25, 2019, with respect to a workplace located at Sweetbriar Village, Building 300 Warrington PA.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) 1754 Gibson Rd. Salem Manor Bensalem - On or about August 21, 2020, employees were performing roofing work on a steeply pitched roof, 30 feet above grade. The employee's rope grabs were too long allowing employees to free fall more than 6 feet, making contact with the ground.
