On or about August 20, 2020, located at 3900 Bixby Road, Groveport, Ohio: the employer did not develop a documented energy control procedure (lockout/tagout, LOTO) for, but not limited to the FR-100 stencil machine, 2.0 Coding and Marking System, Rea Jet print head.  The system was not de-energized prior to the troubleshooting/commissioning, which resulted in the fingertip amputation of an employee who conducted the task, when his finger was caught-in between the mount and guide rail.

On or about August 20, 2020, located at 3900 Bixby Road, Groveport, Ohio: the employer did not ensure that the FR-100 stencil machine, used to stencil information onto railroad wheels, was shut down prior to conducting the troubleshooting of the new installation of the 2.0 Coding and Marking System, Rea Jet print head.  An employee was therefore exposed to the caught-in between hazard of the mount and guide rail and amputated his fingertip as a result.  Within the past five years, the employer had been cited for the same hazard in a different inspection.  Amsted Rail Company, Inc., was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, which was contained in OSHA inspection number 1202344, citation number 1, item number 4 and was affirmed as a final order on January 10, 2019, with respect to a workplace located at 3900 Bixby Road, Groveport, Ohio 43125.
