29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph: (a) On or about 06/03/2020 - Cooling Tower Switchgear building, D4 cubicle, the 4000-Volt surge arrester panel did not have a protective cover installed to prevent contact with energized conductors.

29 CFR 1926.416(a)(3): Employer did not advise employees of the location of energized electrical power circuits, the hazards involved, and the protective measures to be taken: (a) On or about 06/03/2020 - Cooling Tower Switchgear building, D4 cubicle, employees installing insulating electrical boots on de-energized 5kV cubicles were not informed of the location of similar switchgear cubicles in the building area that were energized.
