29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): a) Board Machine Number 3, Georgia Pacific Naheola Mill; On or about August 26, 2020 and at times prior; the employer exposed employees to amputation hazards in that employees were required to adjust the edge wipe on the third coater tending side while the machine was energized The employer failed to follow the required sequence of actions as set by the standard when equipment is energized for testing or positioning.
