29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.  At the site - Building 1900 North West Stairwell 3rd Floor, an employee was sitting in front of a an opening unguarded by a guardrail system that was missing the mid rail, creating a 20 fall hazard, on or about 8/25/2020.

29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:  At the site - building 1900 North West Stairwell an employee was using a A-Frame ladder on stairs not at the same elevation level, creating a 6 feet fall hazard, on or about 8/25/2020.
