29 CFR 1910.23(b)(10): The employer did not ensure any ladder with structural or other defects was immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with 1910.145 and removed from service until repaired in accordance with 1910.22(d), or replaced.  Tool Trailer behind the Lippmann Jaw Crusher: On or about August 28, 2020 an access ladder which was previously a rolling ladder was used to make entry and exit to the Tool Trailer and was found to have damaged steps.

29 CFR 1910.28(c)(3): The employer did not barricade the area into which objects could fall, prohibiting employees from entering the area, and keeping objects far enough away from an edge or opening to prevent them from falling to a lower level:  Tool trailer behind Lippmann Jaw Crusher: On or about August 28, 2020 the left side trailer door where employees enter and exit the tool trailer was found hanging by the top hinge only. This created a potential struck by hazard for employees walking near or under the area.

29 CFR 1910.25(b)(1): The employer did not ensure handrails, stair rail systems, and guardrail systems were provided in accordance with 29 CFR 1910.28.  a) Lippmann Jaw Crusher Conveyer Stair: On or about August 28, 2020, the stairs leading to the Lippmann conveyer had damaged stairs and missing mid rails.   b) Tower ladder area: On or about August 28, 2020, stairs leading to the Tower Ladder were missing handrails.  c) Sandvik conveyer area: On or about August 28, 2020, stairs missing handrail and platform was missing mid rails.   Herb Holden trucking, Inc. the parent company of Recycled Concrete Products of Connecticut, Inc.  was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1910.29 (f)(1)(I), which was contained in OSHA inspection number_1336547_, citation number __1__, item number __1b _and was affirmed as a final order on December 28, 2018, with respect to a workplace located at 31 Apothecaries Hall Road, Broad Brook, CT 06016.
