OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to an ignited flammable compressed gas cylinder that was under pressure:  (a) Lab:  On or about 8/27/2020, employees were exposed to fire hazards during the set up of a Testex 45 Degree Flammability Tester, model # TF310, serial # US20052901, using a 20 pound butane tank and a Fisher FS-76CH-743 LP gas regulator, which caused the Testex Tester to malfunction from over-pressurization of the gas regulator, resulting in an employee injury.
