29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  Snake Creek Water Treatment Plant  located at 1162 Crossplains Hulett Road Carrollton, GA 30116, Excavation behind office building: On or about 08/27/2020 working in and around an excavation that measured approximately 10-feet deep by 15-feet wide by 25-feet long, were not provided training to recognize the hazards associated with trenching and excavation and the means to control or eliminate those hazards.

29 CFR  1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:   Snake Creek Water Treatment Plant  located at 1162 Crossplains Hulett Road Carrollton, GA 30116, Excavation behind office building: On or about 08/27/2020, the employer exposed employees to cave-in/struck-by hazards while working inside an excavation which was approximately 10-feet deep, in that a safe means of egress was not provided to enter or exit the excavation where employees were preparing for sewer pipe installations.

Snake Creek Water Treatment Plant  located at 1162 Crossplains Hulett Road Carrolton, GA 30116, Excavation behind office building: On or about 08/27/2020, the employer exposed employees to cave-in/struck-by hazards while working inside an excavation which was approximately 10-feet deep, in that a cave-in protection system was not provided to employees engaged in sewer pipe installation activities.
