PDQ Manufacturing Inc.  in Ellijay GA: Employer did not maintain OSHA 300 logs or OSHA 301 incident report forms for work place injuries for five years, on or about August 31, 2020.  a.	On or about August 31, 2020 the employer did not maintain OSHA 300 logs or the equivalent 301 for 2019. Employer documented five (5) cases of work related injuries from 1/1/2019 to 4/1/2020 on an OSHA 300A summary log. b.	On or about August 31, 2020, the employer did not maintain OSHA 301 document for splash incident that occurred on August 26,2020 where an employee had days away from work in relation to the incident.
