Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed by a stone slab: a) At the yard outside the production bay on or about August 18, 2020, and prior thereto, employees loading slabs with the 6 cm Boom Lift Clamp (Aardwolf Slab Lifter AL60 Product Code: AL60, Serial No. 007413) the employer failed to ensure that the employees knew the right side of the boom lift clamp to be used when loading onto an A-frame.
