29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Working Yard, 1831 Broadway, Saugus, MA a) On or about 9/1/2020, employees conducting maintenance on a Wirtgen W220 pavement grinding machine operated the machine during the cleaning process when an employee was injured by contacting an operating part of the machine. Procedures developed for the lockout of the Wirtgen W220 lacked specific instruction as to how the machine is to be locked out and re-energized.

29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. a) On or about 9/1/20, an employee conducted work on a Wirtgen W220 milling machine that required lockout/tagout without adequate training on how to recognize the hazardous energy, and how to isolate the energy sources. A second employee was injured when the machine was re-energized for a step in the cleaning process.
