29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about September 2, 2020 an employee entered a 4.8 foot deep trench without a safe means of access, exposing the employee to the hazards associated with no safe access. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

On or about September 2, 2020 an employee entered a 4.8 foot deep trench where the spoil pile was not placed two feet from the excavation edge, exposing the employee to struck-by hazards.
