1926.502(k)(4) The implementation of the fall protection plan shall be under the supervision of a competent person. 29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: Morton Buildings. Inc., conducting work at 26321 Hull Prairie Road, Perrysburg. OH 43551: On or about August 31, 2020. the competent person onsite laid a board that had not been glued/nailed while the employee working in that area was on break and did not ensure that said employee was tied off or otherwise protected from fall hazards while installing the plywood deck in the loft or the barn at the residential property after returning from break. Employees were exposed to the fall hazards of approximately 14 feet from the plywood deck of the loft to the ground below.

29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself: A) Morton Buildings, Inc., conducting work at 26321 Hull Prairie Road, Perrysburg, OH 43551: On or about August 31, 2020, the employer did not ensure that employees were using the Guardian retractable lifelines in accordance with its design. Employees did not connect the Pelican hooks of the Guardian retractable lifelines to an independent anchorage point when using it as a fall protection device while installing the plywood deck in the loft of the barn at the residential property. Employees were exposed to the fall hazards of approximately 14 feet from the plywood deck of the loft to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: A) Morton Buildings, Inc., conducting work at 26321 Hull Prairie Road, Perrysburg, OH 43551: On or about August 31, 2020, the employer did not ensure that employees were adequately trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection installing the plywood deck in the loft of the barn at the residential property. Employees did not use appropriate fall protection and were exposed to the fall hazards of approximately 14 feet from the plywood deck of the loft to the ground below.
