29 CFR 1926.651(b)(3): When excavation operations approach the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means:    a) jobsite at 4301 South Shore Drive South in Erie, PA. On or about September 2, 2020, a Bobcat E55 mini excavator was used to dig out a trench adjacent to a natural gas line. The excavator bucket struck the gas line, causing a gas leak.     NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED

29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as may be necessary to comply with this part:    a) jobsite at 4301 South Shore Drive South in Erie, PA. On or about September 3, 2020, did not initiate and maintain a safety and health program, in that the company did not provide its employees with general safety and health rules.        ABATEMENT CERTIFICATION REQUIRED:  Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
