This practice created a condition exposing them to fall hazards of greater than 6 feet to the ground below in that: (a) Four (4) employees on Building 3 ceiling joists without personal fall arrest systems being worn. (b) Two (2) employees on Building 3 ceiling joists wearing personal fall arrest systems without being tied off.
