29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves.      On or about August 18, 2020 the employer failed to ensure that effective guarding was in place on the shaft of the output side take up reel of the Gribetz Paragon 2 border quilter. The shaft did not have a smooth edge or contain a cover or sleeve.
