On September 4, 2020, located at the House 5 breaker panel, the employer did not effectively close an unused opening on a 120v/208v electrical service panel in that a breaker filler plate was missing on the interior panelboard of an I-T-E circuit breaker box, exposing employees to electrical hazards.

29 CFR 1910.305(g)(2)(iii): (Flexible cords and cables -- Identification, splices, and terminations) Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. On September 4, 2020, located at watering booms near row 10 in House 8, the employer did not provide strain relief to in-use electrical cords hanging off the booms, in that electrical cord outer jackets were significantly damaged and not protecting interior conductors and insulators from water spray, exposing employees to electrical hazards.

A receptacle installed in a wet or damp location shall be suitable for the location. On September 4, 2020, located throughout Houses 1B, 4, 6, and 7, at least 16 electrical outlets were installed in wet or damp locations (near watering booms or under a missing ceiling) that did not have weatherproof covers; were significantly damaged with exposed internal components; and/or not protected from weather and/or water sources, exposing employees to electrical hazards.
