(a) Location - Jobsite On or about September 4, 2020, employees were exposed to shock and electrocution hazards when a cord they were using was missing a grounding pin.  (b) Location - Jobsite On or about September 4, 2020 employees were exposed to shock and electrocution hazards when a cord they were using had damaged insulation at the cord ends and in the middle.  The damage to the insulation exposed live conductors.
