29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location - Jobsite: On or about September 4, 2020 employees were working on a roof. The fall hazard was approximately 14 feet and they dd not have fall protection.

29 CFR 1926.403(b)(1): The employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees. (a) Location - Jobsite On or about September 4, 2020, employees were exposed to shock and electrocution hazards when a cord they were using was missing a grounding pin. (b) Location - Jobsite On or about September 4, 2020 employees were exposed to shock and electrocution hazards when a cord they were using had damaged insulation at the cord ends and in the middle. The damage to the insulation exposed live conductors.
