29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: a) Assembly: Hand tools were not provided and utilized to prevent the operator from coming into contact with moving blades. On 9/4/2020 an employees thumb was amputated when it was pulled into the moving blade of a DeWalt DW716 12" double bevel compound miter saw, while making a one to one and a half inch cut into a piece of 3/4 inch PVC pipe. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
