29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) South of repair shop doors at 6327 Tower Avenue, Superior, WI; On or about September 8, 2020, an energy control program was not established prior to performing servicing and maintenance activities including; adjusting slack adjusters, checking the air compressor, and changing a brake chamber on a Kenworth brand, model T800 quad-axle dump truck (DOT # 2173607, company asset # K-12).
