29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  The energy control procedures utilized for de-energizing the mechanical power presses (Press #2, 3, 4, 5, 6, 8,9, 10, 13) did not include specific procedural steps to ensure the flywheel came to a complete stop, press slide positioned at bottom dead center of its stroke, air pressure was discharged and turned off and locked, and the spring pressure was neutralized.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  By the V & O Mechanical Power Press #9, a single metallic outlet box with knockouts was hanging by a type of tape not being used in accordance with the listing and labeling which required the box to be surface mounted exposing employees to electrical shock hazards.

The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  The employer did not indicate which machine or equipment in which the energy control procedure was utilized for the periodic inspections conducted on October 22, 2019 and October 20, 2020.
