29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      A) Associated Contractors, on or about September 08, 2020 at the address of 968 South Main Street, Meadville, PA: On or about 9/9/2020, employees were not protected from falls while exposed to fall hazards ranging from 9-feet up to 18-feet. Employees were observed laying OSB on the roof of the structure and were not using any type of fall protection.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    A) Associated Contractors on or about September 08, 2020 at the address of 968 South Main Street, Meadville, PA: On or about 9/9/2020,  an employee was utilizing a 600s JLG Boom Lift as a crane to hoist materials to the roof of the structure. The employees operating the aerial lift have not been trained on how to properly use the equipment and the requirements noted in the manufacturers manual. An employee was observed hoisting material from the bottom of the lift without being trained.
