On or about September 10, 2020, at the above addressed jobsite, employees conducting a water connection to a water main, in a trench approximately seven (7) feet in depth did not have a safe means of egress.

29 CFR 1926.652(a)(1): Each employee in an excavation is not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): a. On or about September 10, 2020, at the above addressed jobsite, employees performing water connection operations in a trench that measured approximately seven (7) feet in depth by approximately twelve (12) feet in width on the west end of the trench, approximately nine (9) feet in width on the east end of the trench, by twenty-five (25) feet in length, were not provided with cave in protection, thereby exposing the employees to cave in hazards.
