29 CFR 1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.    On or about September 9, 2020, at an auto dealership renovation site located at 2920 N. Aspen Ave in Broken Arrow, OK, an employee was allowed to work in the basket of an aerial lift without attaching a proper lanyard to the anchorage point in the lif, exposing the employee to the hazard of falls.  Arrow Construction Resources, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 1443818, citation number 1, item number 1, and was affirmed as a final order on February 12, 2020, with a final abatement date of February 5, 2020, with respect to a workplace hazard at 1651 N. 9th Street in Broken Arrow, OK 74012.
