29 CFR 1910.147(c)(4)(ii)(D):The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control: a.) Welch's Food, Inc. - North East, PA - On or about 9/11/2020 - Anhydrous Ammonia System: The lockout procedure including but not limited to the procedure associated with SOP MS-EV-11E did not include a verification of de-energization of isolation step. The specific lockout/tagout procedure did not contain specific techniques, instructions, and/or procedures to inform and guide the authorized employee to verify that all energy sources and associated components were deenergized. b.) Welch's Food, Inc. - North East, PA - On or about 9/11/2020 - Anhydrous Ammonia System: The lockout procedure including but not limited to the procedure associated with SOP MS-EV-11W did not include did not include a verification of de-energization of isolation step. The specific lockout/tagout procedure did not contain specific techniques, instructions, and/or procedures to inform and guide the authorized employee to verify that the all energy sources and associated components were deenergized. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
