29 CFR 1910.22(a)(3): (3) Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. Employees were exposed to slip and fall hazards. Employees working in the facility's kitchen and dining area were exposed to slip and fall hazards. The employer did not ensure that walking-working surfaces were kept free from water accumulation from ceiling leaks.

29 CFR 1910.22(d)(2): (2) Hazardous conditions on walking-working surfaces were not corrected or repaired before an employee uses the walking-working surface again. If the correction or repair cannot be made immediately, the hazard must be guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired; Employees were exposed to trip and fall hazards. Employees working at the facility's kitchen area were exposed to trip and fall hazards. Missing floor tiles and frayed carpet were causing a tripping hazard. The employer did not ensure that walking-working surfaces were repaired or guarded.
