29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about September 11, 2020, during the installation of roofing material for the construction of a municipal building located at 4759 Mayfield Road, South Euclid, Ohio, the employer did not ensure employees working from a 5/12 pitch roof were protected from various fall heights of approximately 11 and 18 feet, by utilizing a guardrail system, safety net system and/or personal fall arrest system.
