29 CFR 1926.600(a)(6)(iv): A person was not designated to observe clearance of the equipment and give timely warning for all operations where it is difficult for the operator to maintain the desired clearance by visual means: On or about September 10, 2020, employees were exposed to an electrocution hazard while performing a cement pour on a new bridge located at 11321 Broadway, Garfield Heights, Ohio 44125. An employee was fatally injured while acting as the hose man on a Putzmeister cement Truck and the employer did not ensure to designate a person to observe the clearance to the energized power lines.
