29 CFR 1910.269(a)(2)(iv):The employer did not determine that each employee is complying with all required safety related work practices by regular supervision and annual inspections: (a) On or about September 2, 2020, and at times prior, the employer did not conduct annual inspections, or have regular supervision of work practices to ensure its employees were performing safe work practices while servicing electrical components at the worksite located at 650 Sybilla Lane, Colorado Springs, CO. An employee suffered serious injuries sustained by an arc flash that occurred while servicing that equipment. Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.269(m)(3)(ii):The employer did not ensure that all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be deenergized are open. The employer didn't render such means inoperable, unless its design does not so permit, and then ensure that such means were tagged to indicate that employees are at work: (a) On or about September 2, 2020 an employee was seriously injured by an arc flash that was created by the employee coming into contact with live electrical components while servicing electrical equipment that was not de-energized located at 650 Sybilla Lane, Colorado Springs, CO. 29 CFR 1910.269(l)(3)(iii):The employer did not ensure that no employee approaches or takes any conductive object closer to exposed energized parts than the employer's established minimum approach distance, of 36 inches: [A] The employee is insulated from the energized part (rubber insulating gloves or rubber insulating gloves and sleeves worn in accordance with paragraph (l)(4) of this section constitutes insulation of the employee from the energized part upon which the employee is working provided that the employee has control of the part in a manner sufficient to prevent exposure to uninsulated portions of the employee's body), or [B] The energized part is insulated from the employee and from any other conductive object at a different potential, or [C] The employee is insulated from any other exposed conductive object in accordance with the requirements for live-line bare hand work in paragraph (q)(3) of this section. (A) The Minimum Approach Distance (MAD) requirements for employee protection were not followed as required. (a) On or about September 2, 2020 an employee was in the process of servicing electrical components when he was seriously injured by an arc flash resulting from the employee making contact with live electrical components. 29 CFR 1910.269(l)(8)(v): The employer did not ensure that each employee exposed to hazards from electric arcs wore protective clothing and other protective equipment that had an arc rating greater than or equal to the heat energy estimated under paragraph (l)(8)(ii) of this section whenever that estimate exceeded 2.0 Cal/cm2: (a) On or about September 2, 2020 an employee was seriously injured by an arc flash resulting from the employee making contact with live electrical components without the benefit of adequate protective clothing or additional protective equipment being in place, including, but limited to, rubber insulating gloves with protectors and head and neck arc protection.
