29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) Coastal Diagnostics 4674 Airport Blvd #C, rear of building: On or about August 24, 2020, the employer allowed employees to work in close proximity to energized 208 volt conductors while drilling a pilot hole for a knockout tool using a drill with a bit and chuck that were capable of damaging the insulation of the conductors without first de-energizing and grounding the circuits or effectively guarding the conductors using insulation or other means of protection capable of resisting damage from the drill chuck and bit.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Coastal Diagnostics 4674 Airport Blvd #C: On August 24, 2020, the employer had an employee suffer an electric shock while drilling an access hole in an electrical disconnect box requiring hospitalization. The employer failed to report the work-related hospitalization to OSHA within twenty-four (24) hours.
