29 CFR 1926.602(c)(1)(vi):Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks: a) 95 South Chenango Street, west side of building, on or about 8-24-20: One employee was discharging material via a Grout Placement System (Grout Hog) that was being hoisted by a rough terrain telehandler that was loaded beyond the manufacturer's safe capacity limits, causing the telehandler to tip forward and pinning the employee between the wall and the unit.
