29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: On or about September 9, 2020, inside a structure under construction located at 1420 West 23rd Street in Miami, Florida, employees were exposed to a fall hazard of approximately 8 - feet, 6 - inches when working near a wall opening leading to an elevator shaft while not being protected from falling into the shaft after opening an unlocked and/or secured door in front of the shaft opening.
