At worksite located at 1102 Fayette, NKC MO -- Forklift trucks did not have a daily inspection worksheet in place. This exposed employees to operating forklifts with mechanical problems and/or failures, resulting in caught in, crush by and struck by hazards. This was documented on or about 09/15/2020.

29 CFR  1910.303(b)(1)(iv):Electric equipment was not free from the recognized hazard of damaged insulation.  a) At worksite located at 1102 Fayette, NKC MO -- Orange extension cord used to power the scale in the warehouse had the outer insulation cut exposing interior wires. This exposed employees to electrical shock and/or burns. This was documented on or about 09/15/2020.  b) At worksite located at 1102 Fayette, NKC MO -- The charging cables for the forklifts were damaged with what appeared to have tape over bare wire on the positive cable, and exposed wires on the negative. Additionally there was obvious damage to the outer insulation (knicks, indents, tape, etc). This was documented on or about 09/15/2020.
