OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees  a.  On or about September 02, 2020, located at 1455 James Parkway, Heath, Ohio:  the employer failed to ensure that employees were not exposed to hazardous conditions by caught-in and struck-by hazardous operations, when two temporary employees operated the KONECRANES, Model A24835, Serial number 3130512, at the Standard Duty Finishing Line (SDFL).  Two temporary employees in training were left at the uncoiler machine location of the SDFL, unsupervised by the designated trainer while they operated the crane with the wireless remote.  The assigned and designated trainer by management, left the two trainees to continue and conduct the hazardous task of positioning the mult coil onto the uncoiler machine.  The temporary employees were non-designated, nor qualified employees, when they operated the crane and were exposed to struck-by hazards as they completed the task.  As a result to the lack of training and supervision, an employee was struck-by and had his right hand caught-in between the uncoiler machine and mult coil, which resulted in a crush injury to his right hand.    Samuel, Son & Co. (USA) Inc., was previously cited for a violation for this Occupational Safety and Health standard, which was contained in OSHA inspection number 1459672, citation number 1, item number 2 and was affirmed as final order on August 03, 2020, with respect to the workplace located at 1455 James Parkway, Heath, Ohio 43506.
