29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):   (a)Rapid Construction LLC at 6812 Wedgewood Court, Rapid City, SD 577702: On or about and at times prior to September 16, 2020, the employer did not ensure that employees conducting work in an excavation were protected from cave-in hazards. Employees were installing a new city water main in an excavation that varied in depth from approximately seven feet to eight feet which lacked an adequate protective system, such as sloping, benching, or shielding system. This condition exposed employees to cave-in hazards within an excavation.

This condition exposed three employees to injuries associated with  rock and debris falling into the excavation.
