29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: At a facility located at N4158 19th Road, Pound, WI 54161; Machine Operator employees did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available, and the methods or means necessary for energy isolation and control when the employees were potentially exposed to the energization and/or residual energy including but not limited to the: a) Stringer Operator was not trained as an authorized employee to recognize the applicable methods and means necessary for isolating the electrical and pneumatic energies or the application of lockout devices during setup and adjustments of the Stringer (Viking M# Turbo 505) b) Trim Saw Operators were not trained as authorized employees to recognize the applicable electrical and pneumatic energies, their magnitudes or the methods and means necessary for isolating the electrical and pneumatic energies or the application of lockout devices during saw blade replacement of the Trim Saw (West Plains M# WPRS-MTS) c) Rip Saw Operator was not trained as an authorized employee to recognize the methods and means necessary for energy control by verifying that deenergization and isolation were accomplished before saw blade adjustments and saw blade replacement of the Rip Saw (Cornell M# 6-36)

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): At a facility located at N4158 19th Road, Pound, WI 54161: b) Two Trim Saw Operators were exposed to machine hazards associated with potentially rotating parts/saw blades when conducting blade replacement of the Trim Saw (West Plains M# WPRS-MTS). The employer did not ensure that employees implemented energy control application steps prior to allowing them to remove and replace saw blades. The saw was stopped from the night before and the operators opened the interlocking covers of the saw before climbing into the machine to unbolt and remove/replace saw blades. The Trim Saw was turned off but not isolated from electrical and pneumatic energy sources prior to the employees engaging in servicing/maintenance activity. c) A Rip Saw Operator was exposed to machine hazards associated with potentially rotating parts/saw blade when conducting blade replacement of the Rip Saw (Cornell M# 6-36). The employer did not ensure that employees implemented energy control application steps prior to allowing them to remove and replace saw blades. The operator stopped the machine, disconnected the power source, applied a lockout device but did not verify that deenergization and isolation had been accomplished by pushing the machine's start button before reaching into the machine to remove/replace saw blades. The Rip Saw was turned off and isolated but was not verified as such prior to the employee engaging in servicing/maintenance activity. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.
