29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, and flying chips.  49 Hartford Ave. W, Mendon, MA: On or about July 21, 2020 one or more methods was not utilized to prevent and employees hands or arms from entering the guillotine entrance to log splitter exposing employees to amputation hazards from the descending cutting head while splitting fire wood.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  49 Hartford Ave. W, Mendon, MA: On or about July 21, 2020, the employer had not notified OSHA of a work related accident which involved the amputation of an employee's thumb.
