29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe. On the east side of the construction site, employees operating skill saws with the lower guards wedged in the up position while cutting cornice material were exposed to amputation hazards: a) Makita portable skill saw #1 lower guard wedged in the up position. b) Makita portable sill saw #2 lower guard wedged in the up position. c) Makita portable skill saw #3 lower guard wedged in the up position.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: On the east and west side of the construction site, employees performing cornice work from jack scaffolds without fall protection, were exposed to fall hazards approximately 12 feet to the lower level. a) Jack scaffold on the west side no guardrails or personal fall arrest systems provided. b) Jack scaffold on the east side no guardrails or personal fall arrest systems provided.
