(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling. a) On or about September 13, 2020, an employee was working from an elevated surface/platform, ice blasting soot and debris from an overhead exhaust hood. The employee was not utilizing a fall protection device and was exposed to a fall of 14 feet 7 inches.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) 512 West Virginia Ave, Parkersburg: On or about September 13, 2020 an employee fell from a height of 14 feet 7 inches while working from an elevated platform without fall prevention devices. The injury resulted in an impatient hospitalization that was not reported to OSHA until September 16, 2020.
